Adirondacks ACO Launches Mobile Web Site

Adirondacks ACO, one of Medicare’s newly accepted Accountable Care Organizations (ACOs), today announced the launch of its informational website, AdirondacksACO.com.  The new mobile site is designed to provide the more than 25,000 Fee-For-Service Medicare beneficiaries in the region with more information on the role of an Accountable Care Organization in their patient care.

Adirondacks ACO is an accountable care organization (or ACO) working with Medicare to make it easier for doctors and hospitals to collaborate in order to provide high quality coordinated care to their patients, while helping to slow the growth of health care costs.  The network of providers currently includes more than 450 participating primary and specialty clinicians throughout Franklin, Clinton, Essex, Hamilton, Warren, Washington and Northern Saratoga Counties of New York and Grand Isle and Chittenden Counties of Vermont.

An evolution of the Adirondack Region Medical Home Pilot, which focuses on preventive care and enhanced coordination of chronic conditions, the Adirondacks ACO is focused on improving community health by delivering high quality through increased coordination of care for its patients.  Participating primary care providers are from Fletcher Allen Partners (CVPH Medical Center, Elizabethtown Community Hospital, Fletcher Allen Medical Group), Adirondack Health/AMC, Alice Hyde Medical Center, Glens Falls Hospital, Irongate Family Practice, Hudson Headwaters Health Network and independent community primary care practices.

The Adirondacks ACO is a 9-county accountable care organization (ACO) comprised of an extensive network of providers to coordinate health care for Medicare beneficiaries.
